Mosaic Community Care Inc. (Mosaic) is a not for profit organisation based in
the Perth suburb of O’Connor, Western Australia.
Mosaic is a multi-disciplinary service provider for people with an
intellectual and/or physical disability. Services include full time care in
supported accommodation together with individualised services tailored for
people living in their own homes within broader community.
Established in 1992, Mosaic has grown to be a dynamic organisation
employing approximately 150 staff in a variety of locations across the Perth
Mosaic currently provides services to fourteen supported homes in the
greater south metropolitan area: Medina, Calista, Bertram, Baldivis (2), Warnbro,
Waikiki, Port Kennedy, Samson, Spearwood, Canning Vale, Thornlie, Kenwick
(2) and Brookdale (2).
Part of Mosaic’s ongoing strategy includes additional opportunities for
people in our care such as independent living units, two of which are
already in place at residential properties Baldivis and Spearwood, with
further redevelopment and independent living units scheduled in the near
future for Brookdale and Safety Bay.
Mosaic also provides contemporary, flexible, innovative and
strength-based services and support to persons with a disability who live in
their own homes, within their communities. Our Individualised Services
Program covers the Perth Metropolitan area and embraces a collaborative,
person-centred approach, involving each individual, their family and friends
who can assist in devising an Individual Plan that outlines goals and
aspirations. These plans are “live” documents and although annual reviews
are held, are “needs-led” and can be reviewed at any time as a person’s
needs or interests change. Examples of creative, innovative and fun
supports, to date, include organising holidays to Rome, Bali and Melbourne,
as well as local support accessing fairs, fun days, the Christmas Parade in
Perth, shopping and the local gym and library.
